100 Days Program

Virginia Cardiovascular Specialists (VCS) has partnered with Heartbeat Health to offer an innovative program for our patients who are leaving the hospital after a hospital stay.

The 100 Days Program involves routine virtual heart health monitoring and virtual televisits as needed for the one hundred days immediately after leaving the hospital.

If your VCS provider believes you may benefit from this program, you will be contacted following your hospital discharge via email, mail and phone.

There is no cost to be enrolled in this program and the goal is to help you recover in the comfort of your home.

Who is Heartbeat Health?

Heartbeat Health is a Virtual-First Cardiology company with a team of Cardiology specialists that has partnered with your physician at Virginia Cardiovascular Specialists to deliver you tailored care and monitoring post hospital discharge.

How did I get enrolled?

Your VCS physician identified you as someone who was recently discharged from the hospital and believes you may benefit from this virtual monitoring program.

What do I need to do?

Heartbeat Health will contact you via phone to get you enrolled in the program. You can also enter your information on the form below to expedite that process.  You will receive instructions on your phone on how to start your first virtual televisit with the Heartbeat Health care team, who work in partnership with your VCS physician.

You can expect regular virtual televisits along with blood pressure and weight monitoring, as needed. You simply need to attend these virtual televisits and log your vitals as instructed by the Heartbeat Health care team.

Will I be billed?

There is no cost to you to be involved in this program.
